Physical therapy telehealth software does more than provide a video connection for a remote appointment. The clinician still has to listen to the patient’s concerns, observe movement, explain the treatment, assign exercises, and record the encounter. 

Virtual physical therapy combines live consultations with tools used before and after the appointment. The format works for some clinical situations, while others require in-person care.

The language around it varies. Physical therapy telehealth refers to the delivery of remote clinical care, while telemedicine software for physical therapy is the technology used to support it. During real-time video consultations, the clinician can use screen sharing to review an exercise, anatomical image, or educational resource. After the Appointment

Once the call ends, the clinician documents the encounter through the clinic’s established workflow. The prescribed home exercise program is then delivered through the patient-facing application, giving the patient continued access to exercise videos, dosage, and instructions.

Between virtual physical therapy appointments, secure messages, symptom reports, and patient-reported outcome measures provide additional information for follow-up.

Synchronous Telehealth, Asynchronous Care, HEP, and RTM Are Not the Same

Feature lists often blur these functions. In clinical practice, however, they remain distinct. A scheduled call, an exercise program sent afterward, and a symptom questionnaire completed later in the week are separate activities.

Using rehabilitation telehealth software to send exercises does not qualify the activity as RTM. Collecting an occasional response also does not qualify as RTM without the work and documentation required under applicable U.S. billing rules.

Integrations can reduce duplicate data entry, although their scope varies by integration. Connecting a physical therapy telehealth platform to an EHR does not give the platform all the functions of the EHR.

When Telehealth Is Appropriate for Physical Therapy and When In-Person Care Is Better

The right appointment format depends on the information the clinician needs to obtain, the patient’s environment, and whether the planned assessment can be completed safely by video.

Appropriate Clinical and Operational Use Cases

Not every follow-up for a patient recovering at home requires a hands-on assessment. A remote visit often centers on exercise review, symptom changes, or observation of how the patient navigates the stairs at home. Physical therapy telehealth works well for these appointments.

The home setting reveals details that are difficult to observe in the clinic, such as how someone gets out of their own chair, navigates a narrow hallway, or positions exercise equipment beside the bed.

Remote visits also reduce travel for patients with limited mobility or long commutes. Few treatment episodes, however, fit neatly into an “online” or “in-person” category. Virtual physical therapy often works best within a hybrid schedule, with the format selected separately for each visit.

Situations Requiring Additional Consideration

Some clinical questions require more than a camera. Palpation, manual treatment, hands-on testing, and certain balance assessments require in-person care.

Technical problems sometimes end an appointment before the assessment is complete. A frozen image disrupts gait observation, poor lighting conceals movement, and limited space creates safety concerns during movement-based activities.

Urgent symptoms call for prompt assessment in the appropriate care setting. Licensure rules, consent procedures, privacy standards, payer policies, and organizational requirements set boundaries for where remote services are delivered.

What Clinics Should Evaluate in a Platform

A controlled demonstration does not always reflect how the platform performs in daily use. Clinics get a more realistic picture by running a complete test appointment. The first question is whether a patient can join the call without staff spending ten minutes troubleshooting.

Staff open the invitation on the type of device a patient would use, check the sound and image, send an HEP, submit an outcome measure, and inspect the resulting documentation. Extra logins, missing records, and awkward handoffs become visible quickly.
